Anaesthetics and Antiseptics: Simpson, Lister and Safer Surgery
Before the mid-nineteenth century, surgery was limited by three connected problems: pain, blood loss, and infection, meaning operations had to be fast and were often fatal. In 1846, ether became the first anaesthetic used in public surgery, but it irritated patients' lungs and was highly flammable; in 1847, the Edinburgh physician James Simpson discovered chloroform's anaesthetic properties, testing it on himself and colleagues at a dinner party, and it quickly became preferred for its more pleasant effects, including during childbirth. Chloroform faced resistance, including religious objections that pain in childbirth was ordained by God, and safety concerns after some patients died from overdoses, but its use gained wider acceptance after Queen Victoria received it for the birth of Prince Leopold in 1853. Anaesthetics solved the problem of pain, but this created a new danger: surgeons, no longer racing against a screaming patient, began operating for longer and cutting deeper, which increased deaths from infection and blood loss until Joseph Lister addressed this separately. From 1865, building directly on Pasteur's germ theory, Lister used carbolic acid spray on wounds, dressings and instruments during operations, and a study of his surgical wards showed death rates from surgery fall from around forty-six per cent to around fifteen per cent, before later surgeons developed fully aseptic techniques, including sterilised instruments and surgical gowns, by the century's end.

Method
- Learn the three separate problems facing pre-anaesthetic surgery, since exam questions often want them identified precisely: pain, blood loss, and infection, and remember that solving one (pain, through anaesthetics) did not automatically solve the others.
- Learn the order and detail of anaesthetic development: ether was first used publicly in 1846 but had drawbacks (lung irritation, flammability); James Simpson discovered chloroform's anaesthetic effect in 1847 by testing it on himself and colleagues, and it became preferred, especially in childbirth, despite religious and safety objections.
- Learn the counter-intuitive link between anaesthetics and antiseptics: because anaesthetised patients no longer needed operations to be fast, surgeons began performing longer, more invasive operations, which INCREASED the risk of death from infection and blood loss, showing that one medical advance could create a new problem needing a separate solution.
- Learn Lister's specific contribution and its scientific basis: from 1865, applying Pasteur's 1861 germ theory directly to surgery, Lister used carbolic acid spray on wounds, dressings and surgical instruments, and figures from his wards showed death rates after surgery falling from around forty-six per cent to around fifteen per cent.
- Learn why Lister's methods were initially resisted, since this supports both 'explain why' and judgement questions: carbolic spray was unpleasant, cracked surgeons' skin, slowed operations down, and some surgeons doubted the still-new germ theory it was based on.
- For a 4-mark question, contrast anaesthetics (solving pain) with antiseptics (solving infection) as two distinct, separately timed breakthroughs addressing two different problems in surgery.

Explain one way in which the impact of anaesthetics on surgery was different from the impact of antiseptics.
- Choose a precise difference: anaesthetics, from 1847, initially made surgery MORE dangerous in one respect, while antiseptics, from 1865, directly reduced deaths from a major surgical danger.
- Support the anaesthetics side with detail: once chloroform removed the urgency of pain, surgeons began performing longer, more invasive operations than before, which increased the risk of blood loss and, especially, infection.
- Support the antiseptics side with detail: from 1865, Lister's use of carbolic acid spray on wounds, dressings and instruments directly targeted infection, and records from his wards show death rates after surgery falling from around forty-six per cent to around fifteen per cent.
- State the difference clearly: anaesthetics solved the problem of pain but, by enabling longer operations, initially made the infection problem worse, while antiseptics were the breakthrough that finally addressed that specific danger directly.
