Medicine in Britain, c1700-c1900: Vaccination, Germ Theory and Public Health

Between c1700 and c1900, medicine in Britain moved from centuries of continuity to genuine breakthroughs driven by named individuals, science and government legislation. In 1796, Edward Jenner developed vaccination, using cowpox matter to give a boy immunity to the deadlier disease smallpox, after noticing milkmaids who had caught cowpox rarely caught smallpox. In 1861, Louis Pasteur published germ theory, proving specific micro-organisms, not miasma, caused disease; Robert Koch then developed methods for identifying the exact bacteria behind individual diseases. Joseph Lister applied this in 1865, using carbolic acid spray as an antiseptic during surgery to cut deaths from infection. In public health, John Snow's 1854 investigation of cholera around London's Broad Street pump showed the disease spread through contaminated water, years before germs were proven to exist. Building on this and Edwin Chadwick's 1842 report on sanitary conditions, Parliament passed a compulsory Public Health Act in 1875, forcing councils to provide clean water, sewers and medical officers of health.

Method

  1. Keep the two strands of this period separate: NEW EXPLANATIONS (germ theory, Koch's identification of specific bacteria) versus NEW PREVENTION (vaccination, public health legislation) - a question may ask about only one strand.
  2. Learn Jenner's vaccination breakthrough precisely: in 1796 he used pus from a cowpox sore to give a boy immunity to smallpox, after observing that milkmaids who caught cowpox seemed protected from smallpox; vaccination was later made compulsory.
  3. Learn the order of the science: John Snow linked cholera to contaminated water in 1854 through observation and mapping, without yet knowing germs existed; Louis Pasteur then proved germ theory in 1861; Robert Koch developed ways to identify specific disease-causing bacteria after that.
  4. Learn how Lister applied germ theory to surgery: from 1865 he used carbolic acid spray on wounds and instruments, reducing deaths from post-operative infection, showing how a new scientific idea directly changed practical treatment.
  5. Learn the public health chain of cause and effect: Edwin Chadwick's 1842 report exposed the link between filthy living conditions and disease; an 1848 Public Health Act only allowed, rather than required, local improvements; growing acceptance of germ theory then helped push through the COMPULSORY Public Health Act of 1875.
  6. For 'explain why' questions on this period, treat science and technology, individuals and government as separate, linked factors: explain how a scientific discovery gave government a reason to legislate, rather than listing the two separately.
  7. For a 16-mark judgement question on this period, weigh how far a named individual's discovery (e.g. Snow, Pasteur, Jenner) caused change against the role of government, chance observation, or existing conditions - a strong answer explains that discoveries usually needed government backing (laws, funding, compulsion) to change the lives of ordinary people.

Worked example

Explain one way in which John Snow's investigation of cholera in 1854 was different from Louis Pasteur's work on germ theory in 1861.

  1. Identify a precise difference: Snow identified HOW a disease spread (through contaminated water) without knowing WHY, while Pasteur later proved the underlying scientific cause of disease itself (specific micro-organisms).
  2. Support the Snow side with detail: in 1854, Snow mapped cholera deaths around London's Broad Street and traced them to a single contaminated water pump, persuading local officials to remove the pump handle, even though he could not explain the biological mechanism.
  3. Support the Pasteur side with detail: in 1861, Pasteur published germ theory, showing through experiment that specific micro-organisms cause disease and decay, disproving the older idea that they appeared spontaneously or that miasma alone was responsible.
  4. State the difference clearly: Snow's work solved a practical, local problem through observation and statistics without a scientific explanation, while Pasteur's work provided the general scientific theory that later research, such as Koch's, could build on.
