Medicine in Britain, c1900-present: The NHS, Genetics and Modern Public Health

In the history of medicine in Britain, the National Health Service (NHS), founded in 1948, gave everyone free healthcare at the point of use, funded through National Insurance and general taxation rather than direct payment, ending a system where access to a doctor had often depended on ability to pay. Alongside this change in how care was organised, science continued to transform what doctors could actually do: in 1953, James Watson and Francis Crick, building on Rosalind Franklin's X-ray diffraction images of DNA, worked out the double helix structure of DNA, laying the foundation for later genetic research, screening and treatment of inherited conditions. Modern public health has shifted from nineteenth-century concerns about clean water and sewers towards preventing lifestyle-related illness, through government-funded screening programmes and public health campaigns targeting smoking, diet and other risk factors, alongside advances in diagnostic technology such as scanning.

Method

  1. Separate the NHS (how healthcare is organised and paid for) from scientific discoveries like DNA (what doctors understand and can treat) - a question may focus on only one of these.
  2. Learn the core fact about the NHS founded in 1948: it provided healthcare free at the point of use for everyone, funded collectively rather than through direct payment by the patient, as part of a wider post-war welfare state.
  3. Learn the DNA breakthrough precisely: in 1953, Watson and Crick worked out that DNA has a double helix structure, building on X-ray diffraction images produced by Rosalind Franklin; this discovery later underpinned genetic screening and research into inherited disease.
  4. Learn how public health priorities changed after 1900: nineteenth-century public health focused on sanitation and infectious disease (clean water, sewers); twentieth and twenty-first century public health increasingly targets lifestyle-related illness (smoking, diet, inactivity) through government campaigns and screening programmes.
  5. For 'explain one way X was similar to/different from Y' questions, compare who PAYS for treatment across periods: contrast the NHS's free-at-the-point-of-use model with earlier periods, when patients paid physicians directly or relied on charity.
  6. For 'explain why' questions about this period, treat government (the NHS, public health campaigns), science and technology (DNA, scanning), and changing attitudes in society (acceptance of state involvement in healthcare after the Second World War) as linked factors, not a simple list.
  7. For a 16-mark judgement question on how far government has been the main driver of change since 1900, weigh the NHS and public health legislation against scientific discoveries, such as penicillin and DNA, that government did not directly cause, before concluding.

Worked example

Explain one way in which access to healthcare in Britain after 1948 was different from access to healthcare in the nineteenth century.

  1. Choose a precise difference: after 1948, healthcare was free at the point of use for everyone through the NHS, whereas in the nineteenth century most people had to pay a physician directly or rely on charity.
  2. Support the nineteenth-century side with detail: a working-class family might rely on a cheaper apothecary, a charitable dispensary, or simply go without a doctor, because consulting a physician cost money they often did not have.
  3. Support the post-1948 side with detail: the NHS, founded in 1948 and funded through National Insurance and taxation, meant a patient could see a doctor or go to hospital without paying directly, regardless of income.
  4. State the difference clearly: the NHS removed the direct link between a patient's ability to pay and their access to treatment, which had shaped who received good medical care throughout the earlier periods of this course.
