The Cholera Epidemic of 1854 and John Snow's Broad Street Investigation
Cholera is a bacterial infection, spread mainly through water contaminated with human waste, that causes severe diarrhoea and rapid, often fatal, dehydration; it first reached Britain in 1831-32 and returned in several later epidemics. During the 1854 outbreak in Soho, London, the physician John Snow, who already suspected water rather than miasma (bad air, the dominant explanation of the time) carried the disease, plotted every cholera death on a street map and traced an unusually dense cluster to a single water source, the public pump on Broad Street. Investigating further, Snow found that workers at a nearby brewery, who drank mainly beer rather than pump water, had escaped the outbreak almost entirely, and that a woman who had moved away from Broad Street still died after having pump water delivered to her new home because she preferred its taste. Snow persuaded local officials to remove the Broad Street pump handle, and separately compared cholera death rates among customers of two different London water companies, one drawing water from a heavily polluted stretch of the Thames and one from a cleaner stretch, finding far higher deaths among the company using dirtier water. Snow's careful, evidence-based method identified the true mechanism of cholera's spread decades before Robert Koch identified the specific bacterium, Vibrio cholerae, in 1883-84.

Method
- Separate WHAT Snow proved (that contaminated water, not miasma, spread cholera) from HOW he proved it (mapping deaths, interviewing residents, and comparing two water companies), since exam questions may test either the finding or the method.
- Learn the Broad Street evidence precisely: Snow mapped cholera deaths and found them clustered tightly around the Broad Street pump; he then found the brewery workers nearby, who drank beer instead of pump water, had barely been affected, and traced one death outside the immediate area to a woman who still had pump water delivered because she liked its taste.
- Learn the water companies comparison as a SEPARATE piece of Snow's evidence: he compared cholera deaths among customers supplied by two rival companies drawing water from different, more and less polluted, stretches of the River Thames, and found far more deaths where the water was more contaminated.
- Remember that Snow's practical success (the pump handle was removed and the outbreak declined) did not immediately win over the medical establishment, since he could not yet explain WHY the water caused illness; miasma theory remained the dominant belief until Pasteur's germ theory in 1861, and the specific cholera bacterium was not identified until Koch's work in 1883-84.
- For a 4-mark 'explain one way X was similar to/different from Y' question, contrast Snow's method (statistical observation and mapping, without knowing the biological cause) with a later or earlier example, such as Pasteur's laboratory proof or the Black Death's total absence of any correct theory.
- For a 12-mark 'explain why' question about Snow's success, use named factors: his own careful method of observation and comparison, the availability of good local death records to map, and, as a limitation, the slow, incomplete acceptance of his findings by officials still committed to miasma theory.
- For a 16-mark judgement question weighing Snow's importance, plan a paragraph on what he achieved (identifying the real cause of transmission through evidence) against a counterpoint (that officials only fully acted on his findings once compulsory public health legislation followed decades later), then reach a supported conclusion.
Worked example
Explain one way in which John Snow's investigation of cholera in 1854 was similar to Edward Jenner's development of vaccination in 1796.
- Choose a precise similarity: both Snow and Jenner reached an important medical conclusion through careful observation of a pattern, rather than through a laboratory experiment revealing an underlying biological cause.
- Support the Jenner side with detail: Jenner observed that milkmaids who had already caught the mild disease cowpox rarely went on to catch smallpox, and tested this pattern by deliberately giving a boy cowpox matter in 1796.
- Support the Snow side with detail: Snow observed that cholera deaths clustered tightly around the Broad Street pump, and that brewery workers who drank beer rather than pump water were largely unaffected, leading him to have the pump handle removed.
- State the similarity clearly: neither man could yet explain the precise biological mechanism (a virus for Jenner, a bacterium for Snow), but both used disciplined observation of who fell ill and who did not to reach a conclusion that later science confirmed.
