Treating Sporting Injuries: The RICE Method and Immediate Care

RICE is the recognised immediate first-aid treatment for most soft-tissue injuries, such as sprains and strains, applied as soon as possible after the injury occurs and typically continued for the first 24 to 72 hours.

Method

  1. Learn what each letter of RICE stands for, in order: Rest, Ice, Compression, Elevation.
  2. Learn the specific action taken for each stage, and the approximate time each is applied for, e.g. ice for 10 to 20 minutes at a time.
  3. Learn why each stage helps: Rest prevents further damage; Ice narrows blood vessels to limit swelling and numbs pain; Compression physically limits swelling; Elevation uses gravity to reduce blood flow and swelling to the area.
  4. Learn that RICE is intended for soft-tissue injuries, sprains and strains, and that a suspected fracture or dislocation should not be treated with RICE alone; it should be immobilised and the performer should seek professional medical help.
  5. Practise applying RICE in order to a given injury scenario, describing the specific action taken at each stage for that injury.
  6. Remember the safety point that ice should never be applied directly to bare skin, since this risks an ice burn; it should always be wrapped, e.g. in a towel or cloth.
  7. When asked to explain a stage's benefit, always name the physiological effect, e.g. blood vessels narrowing, rather than just restating the action.

Worked example

A basketball player rolls their ankle landing from a jump. There is no visible deformity, but the ankle quickly begins to swell and is painful to stand on. Describe how RICE should be applied to this injury in the first 20 minutes.

  1. Rest: the player should stop playing immediately and avoid putting any weight on the injured ankle, to prevent further damage to the ligament.
  2. Ice: a cold pack, wrapped in a cloth or towel to protect the skin, should be applied to the ankle for around 10 to 20 minutes; this narrows the blood vessels near the injury, helping to limit swelling and reduce pain.
  3. Compression: a bandage should then be wrapped firmly, but not too tightly, around the ankle, to help control the swelling that has already begun.
  4. Elevation: the ankle should be raised above the level of the heart, for example by resting the leg on a raised cushion or chair, using gravity to reduce blood flow to the area and further limit swelling.
  5. State the outcome: applying RICE promptly in this order gives the ankle the best early conditions to limit swelling and pain, supporting recovery, though the player should seek further medical assessment if pain, swelling or loss of movement are severe.
